OUR COMMUNITY HEALTH NAVIGATOR

 Our bilingual Community Health Navigator builds trusting relationships in the local Hispanic Community and serves as a liaison between health and social services in the community, to facilitate access to services and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing health knowledge and self-sufficiency trough a range of activities such outreach, community education, informal counseling, social support and advocacy.

Nuestra Promotora de Salud forma relaciones de confianza dentro de la comunidad local de hispanos y trabaja como puente entre los serivicios de salud ofrecidos y la comunidad en general, para facilitar y mejorar la calidad de los servicios de salud ofrecidos. De igual manera, se encarga de llevar a la comunidad temas de salud para incrementar el conocimiento en esta area, mediante charlas, educación comunitaria, terapias informales y apoyo social.
